What does this program provide?
To recognize the birth of a new Puyallup tribal member, a keepsake Pendleton blanket and a diaper bag are provided up to 1-year of birth. Other items may be available depending upon program funding. Am I eligible to receive a blanket and diaper bag for my child? A Pendleton blanket and diaper bag are available to the custodial parent or legal guardian of a Puyallup tribal member child up to 1-year of birth. The tribal member parent must be an enrolled tribal member with a permanent enrollment number. What do I have to do? Fill out an online form for each child you want a blanket and diaper bag for. All applicable information must be included. Upload any required documents. That’s it! What documents are required to be uploaded? You must upload one of the following documents to be eligible:
Can I choose which blanket I want? Yes. Clink the link here to see the current selection of blankets available to choose from. After viewing, please enter the blanket code and name in the submission form below (i.e. ZM609-53436 Woodland Neighbors, etc.). We try to have all the blankets in stock; however there may be times when a particular blanket is not in stock, or has been discontinued. Can I pick the blanket and diaper bag up? You may pick up the blanket at our office in Fife, Washington by calling and setting up an appointment time. Our office is located in the Salish Cancer Center building at 3700 Pacific Highway E, Suite 418, Fife, WA 98424. Can the blanket and diaper bag be mailed to me? Yes. If you are out of the area or unable to come in to our office, we will be happy to mail the items to you at the address you provided. Generally, the items will be shipped within a few days via the US Postal Service. However, the Trust Board will not be responsible for lost or stolen items.
